We want the best for your teeth. That's why we've put together some information about your visit to our clinic, preventative care and other dentistry issues.

Don't forget: If you are exempt from NHS charges, bring your proof of exemption To ensure your visit to our clinic goes smoothly, please bring your proof of exemption every time you visit us.

Preventative care: check-up as often as your dentist recommends You should come for a check-up at the interval recommended by your dentist to make sure that you are looking after your teeth properly and to have their condition checked.

We will ask you to complete a confidential medical history form, along with a social history form at each check up - please allow time to complete these when you arrive.

Failure to Attend Policy

 Due to the high number of failed or cancelled (at short notice) appointments,

we are finding it difficult to maintain our current level of NHS service.

These appointments cause so much lost revenue that we are introducing these terms and conditions applicable to all booked appointments with immediate effect:

 

It is your responsibility to attend all appointments which you make at the practice. Failure to attend two appointments will result in your name being removed from our NHS list at this practice. The same applies to appointments cancelled without 24 hours notice.

 

If you make an appointment as a new patient, and fail to attend then no more appointments will be offered to you.

 

 

 

 

  1. A copy of these terms and conditions is displayed on reception and a copy can be obtained by asking at reception.

 

  1. By booking an appointment with us, it is understood that you agree and accept these terms and conditions.

 

  1. If your name is removed from our list, we shall inform you.

 

  1. If you fail to pay any private charges, we will remove you from our patient list.

 

  1. If you feel there are exceptional circumstances regarding your removal from our NHS list, you should write/email the practice manager.
